In consideration of being allowed to participate in any way in Hands
Across the Water, the undersigned acknoweldges, appreciates and agrees that:
I agree to take the precautions set out by my local health officials.
I confirm that I have not travelled outside of Canada in the past 14
days, experienced COVID-19 sysmotanoms (e.g. fever, difficulty
beathing, etc.) over the past 14 days. Should I experience symptoms, I
will not participate in Hands Across The Water.
The risk of injury from the activites involved in this program is
significant, including inital the potential for permanent paralysis
and death: and while particular rules, equipment and personal
discipline may reduce this risk, the risk of serious injusry does
exist, and:
I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown
EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES or others, and
assume full responsibility for my participation:
I willingly agree to comply with the stated and customary terms and
conditions for participation. if, however, I observe any unusal
significat hazard during my presence or participation, I will remove
myself from participation and bring such to the attention of the
nearest offical immediately: and:
I, for myself and on behalf of my heirs, assigns, personal
representative and next of kin, HEREBY RELEASE AND HOLD HARMLESS
Abilities in Motion, their officers, officials, volunteers, agents
and.or employees, other to conduct he event (all of whom are referred
to as "Releasee's WITH RESPECT TO ANY AND ALL INJURY, DISABILITY,
DEATH, or loss or damange to person or porperty, WHETHER CAUSED BY
NEGLIGENCE OF THE RELEASEES OR OTHERWISE.
BY CLICKING "I accept" AND SUBMITTING THIS
REGISTRATION, I ACKNOWLEDGE THAT I HAVE READ, OR HAVE HAD IT READ TO
ME, THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT,
FULLY UNDERSTOOD ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP
SUBSTANTIAL RIGHTS BY SIGNING AND SIGN IT FREELY AND VOLUNTARILY
WITHOUT ANY INDUCEMENT.
