TICP Office Key Fob Deposit Payment

Permission is provided as per the following agreement for the key holder to access the TICP Office & Community Clinic located at 427 Vaughan Road, Toronto, ON, M6C 2P1.

This key may be accepted under the following conditions:

a)     No duplicates may be created;

b)    Loss of the key fob must be reported immediately to the TICP Administration Manager;

c)     Key fob must be returned to the TICP Administration Manager immediately upon termination of involvement with TICP;

d)    A refundable deposit of $25 must be paid upon receipt of the key fob, payable to TICP by cheque, cash, credit card or e-transfer; this is refundable upon return of the key;

e)     A $25 fee will be charged for each replacement key fob if lost or not returned upon completion of involvement with TICP.

Payment of this fee establishes the acceptance of the terms and conditions for use of this key.

I understand that this key fob will only be used for purposes pertaining to my normal activities within my scope of involvement with TICP. I understand that should I wish to use the space at the TICP Office at times other than the approved scheduled TICP activities, paid rental arrangement must be made separately.
